Referral (if applicable)

Preferred pharmacy phone number

List of prior/current medications and treatments

  • If you are visiting us for a medical appointment but would also like a cosmetic consultation, please request an appointment for both services so that we may reserve enough time to cover everything in detail. We request 24 hours notice should you need to cancel or reschedule your appointment. If this is not provided, you may be charged a $25 fee.

    We understand that unforeseen events sometimes result in a missed appointment and these can be discussed on an individual basis with our office manager.

    All patients 18 years old or under must be accompanied by a parent or legal guardian.
